Cancellation Policy

If you are unable to keep an appointment, we ask that you give 24 hours of notice. This will enable another patient to use the available time. There will be a fee of $50 charged if less than 24 hours notice is given for cancellation.

Cosmetic Consultation Policy

A consultation fee of $200 is taken at consultation appointment – if services are scheduled the $200 is removed from your total.   Cosmetic Registration Form
